Social WorkerSouth Kildare West Wicklow Children’s Disability Network Team (CDNT) Permanent Contract – 35 hours per weekLocation: Kildare TownSocial Worker Salary Scale – €48,624 to max €58,662 DOE In line with the HSE National Programme for Progressing Disability Services , Kare is the Lead Agency for the provision of supports across South Kildare West Wicklow. The successful candidate will join an established Interdisciplinary CDNT, which reconfigured in 2014, providing quality, evidenced based and family-centred supports for children and young people aged 0-18 years, who present with complex needs associated with disability, within an interdisciplinary framework, to support their development to enable active participation in community life. The Social Worker will support children who present with complex needs associated with a disability on Kare’s Children’s Disability Network Team (CDNT) and will work as a key member of the interdisciplinary team providing a family centred quality service to children and their families in line with the National Programme for Progressing Disability Services for Children and Young people. The duties will include direct support/intervention with children and Families.ResponsibilitiesBe responsible for managing and prioritising own caseload in the context of the overall serviceConduct comprehensive assessments of children and families with a disability where requiredDetermine the most appropriate social work methods of practices that would best enhance the quality of life of the person with a disabilityInform the individual with a disability and their family of their rights, entitlements, benefits and servicesAct as an advocate for children and families with a disability where appropriateLiaise with Health Service Executive and relevant voluntary and statutory agencies in relation to the provision of personal and social servicesTo work as part of the interdisciplinary Team and engage in collaborative goal setting as a teamTo participate in case formulation meetings/case reviews as required To contribute to the planning of the service priorities as requiredTo be fully aware of and compliant with current child protection and safeguarding legislation and local policies and procedures RequirementsPossess a relevant Social Work qualification (applications from recent graduates are welcomed)Must be registered with and maintain registration on the Social Worker Register maintained by the Social Work Registration Board at CORUDemonstrate the ability to ensure a high standard of service is deliveredDemonstrate organisational and planning skills with effective interpersonal and communication skillsExperience of working with children and familiesExperience of working in disability servicesFull clean driving licence and use of own car Benefits include:29 days annual leavePaid Maternity/Paternity Leave (subject to service)Rewarding work with flexible hoursEmployee Assistance Programme Contributory Pension Plan Optional Salary Protection PlanPersonal Development, Professional Growth and Career Progression A detailed Job Description is available on request.
