Working both independently and as part of a multidisciplinary team, you will support individuals and families to build resilience, independence, and meaningful social connections.This role is ideal for professionals who value autonomy, collaboration, reflective practice, and real community impact. Key Responsibilities Deliver holistic, person-centred support in line with individual care plans and personal goalsProvide one-to-one and small group support within community settingsPromote independence, dignity, safeguarding, and social inclusionSupport service users with:Community engagement and outreachSchool transport and attendance supportsFamily access and supervised contactDaily living skills (personal care, meals, appointments, routines, and social activities)Work proactively and independently while maintaining regular communication with line managementMaintain accurate, timely, and compliant records and reportsAdhere to all organisational policies, regulatory requirements, and professional codes of practiceContribute positively to a respectful, empowering, and trauma-informed environment What We’re Looking For We are seeking motivated professionals who are passionate about rights-based practice and making a meaningful difference. Essential Criteria:Minimum QQI Level 7 qualification in Social Care, Health Care, or a related discipline (or recognised equivalent)CORU registration (Social Care / Social Work Register, as applicable)Excellent communication, documentation, and IT skillsFull, clean driving licenceGarda Vetting clearanceLegal right to work in Ireland without restrictionStrong understanding of safeguarding, child protection, and professional boundariesAbility to thrive in a dynamic, community-based environment  Job Types: Permanent, Full -time, Part-Time, Ad-Hoc
