Patient Flow Manager – General Wards Clinical Nurse Manager II Grade (Full time, Permanent) The post holder will report to the Assistant Director of Nursing Bed Management and liaises with Ward Managers, Divisional leads, Unscheduled Care Lead, Patient Flow ADON (Emergency Department), Clinical Director, Consultants Purpose of Post:The Patient Flow Manager will play a pivotal role in ensuring the clinical and operational effectiveness of acute patient pathways in Mercy University Hospital. A key focus of the role is to identify barriers to patient investigation and treatment and to ensure all processes and practices relating to the admission, discharge and transfer of patients are in line with best practice. The post holder will work closely with bed management unit, ward managers, consultants and other members of the multidisciplinary team to streamline treatment and discharge processes. The post holder will implement in conjunction with stakeholders strategies for managing patient delays with treatment and discharge. The post holder will be required to provide regular reports to the hospital management team on the delivery of the unscheduled care improvement plan for MUH. The post holder will also support the escalation team and Emergency Department Patient Flow ADON.  Responsibilities - The post holder will be responsible for; Ensure effective streaming of patients through Inpatient wards, between hospital departments and across community services. Liaise and communicate regularly with Admission CNM2, D/C Coordinators, the Emergency Department and Acute Medical Assessment Unit in relation to bed availability.Work with ED Patient Flow ADON to ensure timely transfer of patients from ED.Monitor the Predicted Date of Discharge (PDD) and ensure discharge plan is congruent with PDD. Ensure that whiteboard rounds take place on each ward daily with input from all members of the multidisciplinary team and that boards are updated accordingly. Collaborate and communicate with patients and families to deliver care consistent with best evidence based practice.Ensure the patient is actively participating in the decision making of their care with all appropriate team members if the patient requires clarification regarding disease process, treatment options, or plan of care.Educate and empower ward based staff to expedite all discharges within the operational standard timeframes.Carry out their duties and responsibilities in compliance with the Health and Safety Policies and Statutory Regulations.To assist in the development of the management skills of staff.To motivate team members by agreeing goals and objectives.To promote patient and family health promotion initiatives.Eligibility Criteria: QualificationBe registered in the General Division of the Live Register of Nurses by Nursing and Midwifery Board of Ireland (NMBI)Have a proven track record in General nurse management.Have at least 6 years post registration experienceHave computer and analytical skills.Management course desirablePost registration qualification in Nursing and or Management desirable ExperiencePrevious experience of change management and project delivery Experience of leading quality improvement projectsPossess excellent interpersonal skills and be capable of communicating effectively and efficiently with a wide variety of departments and individuals, including patients and co-workers.Possess the ability to deal with challenging situations and the ability to proactively influence outcomes.Possess good decision-making skills and a capacity for analytical thinking and problem solving with a good knowledge of clinical nursing.Have the ability to provide motivation and team-leadership to all employees within your remit within the clinical areas.Have the ability to work within the constraints of a busy hospital.Possess a strong awareness of the need for confidentiality, discretion, precision and a methodical approach in all matters.Knowledge and experience of the Bed Management function desirable    Closing date for applications is 1pm Monday 12th February 2024The MUH reserve the right to create a panel for this post from which permanent, fixed term and specified purpose vacancies of a full or part time duration may be filled.
