Assistant Medical Officer(Gerontology/Geriatric Medicine) .5 WTEGeriatric or general practitioner experience and qualifications are highly desirable. Leopardstown Park Hospital is a public voluntary hospital with 127 long-term care beds subject to regulation by HIQA. It provides specialist older people’s services in the areas of day services and residential care. The Assistant Medical Officer performs direct clinical care and associated resident/patient medical related activities for a population of older adults in residential care., Other duties may include interdisciplinary meetings, family meetings and other relevant activities. The Assistant Medical Officer may be required to liaise with external and internal stakeholders to ensure optimal delivery and management of care to residents and patients and may participate as required in key internal groups including Integrated Quality & Safety Committee, Medication Safety Committee, Clevis selection Committee and contributes, as appropriate, to the development and implementation of organisational policy in related areas. Purpose of the Post  This post holder will provide direct clinical care and all associated resident medical related activities, including participation in interdisciplinary meetings, family meetings, participate and conduct clinical audits and other relevant activities. The post holder will be required to consult with other members of the interdisciplinary teams and input as appropriate into the development and implementation of organisational policy in related areas Reporting Relationship  This position reports to the Senior Medical Officer and works with the Director Of Nursing (PIC) HSCPs and administrative staff.Principal Duties and Responsibilities  Clinical/Professional Related Responsibilities: The post of Assistant Medical Officer carries responsibility for assisting with the clinical management of the allocated residential, welfare and care services. The Assistant Medical Officer will assist the co-ordination of medical services and will lead/facilitate the functioning of the interdisciplinary team delivering resident care for their assigned areas, in association with the Senior Medical Officer. As part of this process the post holder will attend interdisciplinary meetings, case conferences, family meetings and reviews of patients, residents and clients as required. The Assistant Medical Officer will develop and maintain excellent working relationships and collaboration with external medical providers including, but not exclusively, Consultant Geriatricians, out-of-hour GP medical services, Frailty team, general practitioners, national ambulance service, Emergency Department and Coroner. The post holder is responsible for the maintenance of appropriate medical clinical records in respect of all the people they provide service to. To liaise regularly with Pharmacist on review of residents' medication, charts and retain adequate resident records. This includes medication optimisation for each resident. In conjunction with the Senior Medical Officer and Pharmacist agree a hospital formulary and to use generic drugs where appropriate. To conduct assessments of the capacity of residents where requested and as necessary, including writing reports for court-related processes, capacity assessment, assisted decision making.  The Assistant Medical Officer will offer assistance and advice to staff in case of urgency or emergency at the hospital. In conjunction with the appropriate authority, to conduct such preventative measures e.g., immunisation, vaccination etc. as may be necessary. The Assistant Medical Officer will advise Clinical Nurse Managers/nurses on medical/ clinical care and, where appropriate, will consult with the person' s treating consultant(s).The Assistant Medical Officer will notify immediately the Senior Medical Officer, Chief Executive and/or the Director of Nursing and other appropriate authority of any case of infection or suspected infectious disease in the hospital and to furnish such particulars as may be required about each case.Manage own caseload in accordance with the needs of the post. Participate in teams as appropriate, communicating and working in co-operation with other team members. Initiate and/or participate in research studies as appropriate. Management Related Duties: To comply with the provisions of the Freedom of Information Act, GDPR, data subject access requests regarding requests for release of information to clients and work closely with the Resident Services Manager in this regard To issue to or in respect of any resident under his care in the hospital without payment by or on behalf of the resident, or by the Health Service executive, any certificates which might reasonably be required by such resident in regard to the state of his health in connection with entitlement to benefit under any scheme of Social Insurance or Assistance. To report in writing to the Senior Doctor, Chief Executive, Director of Nursing, any matter affecting the health of people in the hospital, any defects in support services or other arrangements of the hospital, or any defect which may affect the safe care of residents. The post holder will be expected to keep up to date with developments in the medical field particularly regarding the needs which have to be addressed within the hospital. Provide support, advice and directions to staff as required! Engage with the wider team and facilitate team building. Facilitate communication at unit level and within the senior nursing team. Provide staff leadership and motivation which is conducive to good working relations and work performance. Self-Development:To maintain, update and develop knowledge on relevant professional issues and current healthcare trends. To develop and maintain personal practice skills and competencies. Identify your own education and practice needs and take steps to achieve the same. To ensure own mandatory training requirements are fully up to date. Be appropriately skilled in the use of I.T. applications to facilitate delivery of the medical role, including but not exclusively, use of electronic patient records, electronic prescribing, e-referral, mobile Xray referral and reporting & Microsoft office products. Relationships with Other Agencies: The Doctor will maintain a close liaison and a good working relationship with the referring Consultant Geriatricians and other Consultants on a resident specific basis. To coordinate with external medical providers. To refer as appropriate to external medical practitioners, as clinically indicated. Excellent understanding of HIQA Standards for Residential Care and Rehabilitation standards of care and ability to apply these standards to practices in the Hospital as relevant in the role. Enhance and facilitate professional development of staff within organisation policy and service need. The incumbent will be required to establish, maintain, enhance, and develop their professional knowledge, skills, and aptitudes to respond to a developing service situation.    Eligibility Criteria Qualifications and/ or experience   Full registration in the General Register of Medical Practitioners maintained by the Medical Council in Ireland or entitlement to be so registeredPost graduate membership of The Royal College of Physicians or of General Practice would give a distinct advantage. Experience in geriatric medicine is very desirable.
