The Pharmacy provides support across the clinical areas of the hospital and this role is important in making a significant contribution, developing and maintaining best practice in Pharmacy services. This includes the provision of the highest level of safety and regulatory compliance.About the RoleThe Senior Antimicrobial Pharmacist is responsible for promoting best practice prescribing of anti-infective agents through co-ordination of projects and audits in accordance with priorities set by the Infection Prevention and Control Committee, Microbiologists, and Head of Pharmacy. To ensure the safe, effective and cost effective use of anti-infectives. To actively contribute to the monitoring and management of anti-infective drug expenditure through regular analysis and reporting, in order to ensure safe, clinically effective and cost efficient use of drugsResponsibilities include but are not limited to:LeadershipLead on, develop and evaluate anti-infective prescribing to key areas within the hospital, in accordance with the priorities set between the post holder and the microbiologistsProactively develop clinical pharmacy services relating to anti-infectives in line with local and national objectivesProvide leadership for the team and ensure an effective learning environment. Strive to ensure that staff are accountable, responsible and have authority to practice their role effectivelyWork collaboratively with colleagues across pharmacy, medical staff, the multidisciplinary team and management to ensure implementation of local and nationally agreed pharmacy services and objectives, relating to anti-infectivesEngage with and educate stakeholders on issues relating to anti-infective prescribing practice according to local and national policies Service ProvisionPromote best practice prescribing of anti-infective agents through co-ordination of projects and audits in accordance with priorities set by the antimicrobial stewardship group, ensuring safe, effective and cost efficient useActively contribute to the monitoring and management of anti-infective drug expenditure through regular analysis and reporting, in order to ensure safe, clinically effective and cost efficient use of drugsManage budget and identify changes in prescribing practices that may impact on drug budgetUndertake audit and investigations and implement changes and be able to present results at National MeetingsDevelop, implement, monitor and evaluate anti-infective protocols and guidelines and update the sameParticipate in the pharmaceutical aspects of clinical audit to ensure optimal drug usageLiaise with clinicians in smoothing the entry of new anti-infective drug protocols and ensure old practice is phased outGeneral Service ProvisionProvide a high quality, customer focused pharmaceutical service to all Hermitage Medical Clinic in-patients Thoroughly review prescriptions for their clinical appropriateness, safety and legality and conduct medicine reconciliations as per hospital policyDispense and supply drugs to patients as per service demandProvide evaluated pharmaceutical advice and information to all health care professionals and patients of the hospital using on-line databases and primary sourcesHave a working knowledge of pharmacy stock control and ordering systemsScreening of discharge medication prescriptions (TTOs) on the ward, as requiredFor a full job description for this role, please email smaddock@hermitageclinic.ieEssential CriteriaMaster’s Degree in Pharmacy or equivalent Member of the PSI Diploma in Clinical Pharmacy or equivalentMinimum 3 years’ experience as a clinical pharmacist Demonstrate the ability to appropriately recommend, substantiate and communicate therapeutic options and dosage regimens for patients receiving anti-infectivesPractical clinical experience Previous experience in delivering training and educationDemonstrable use of clinical audit to improve practiceExperience including working with Clinicians in an MDT/ward round environmentThe Hermitage Medical Clinic is an Equal Opportunities Employer with a strong commitment to diversity, inclusion and equality at all levels of the organisation.
