Dedicated Community Nurse

TCP Homecare provides innovative healthcare and pharmaceutical services to meet the requirements of a new emerging healthcare environment in Ireland. We are looking for innovative, enthusiastic nursing professionals in the greater Dublin region who demonstrate a high degree of self-motivation while displaying flexibility and ability to adapt to change to join a newly established Dedicated Service on behalf of one of our existing Private Health Insurer Clients, providing a comprehensive high quality standard of nursing care in the community. The community nurse will provide clinical services to patients in an effective and caring manner to a high standard that meets best practice. A key element of the role is the maintenance of accurate electronic patient records and the timely deliverance of reports immediately after a visit back to the clients and head office. The community nurse should see the role as working as part of a team where the nurse uses their clinical, communication and interpersonal skills to provide a seamless service whilst at all times liaising closely with the nursing manager and the client's multi-disciplinary team.   Key Responsibilities:  •       Training Adult and Paediatric patients and/or their carers to facilitate self administration. •       Administration of required IV/IM/SC injections and oral therapies to patients to the highest clinical and ethical standards •       Carry out all required activities including wound care and catheter care. •       Carry out any additional treatments/services as required. •       Timely electronic reporting of each visit as agreed with client and your Nursing Manager •       Maintenance of accurate and detailed records as agreed with client and your Manager •       Recognise and report immediately any untoward events on a patient visit as outlined by your Nurse manager •       Assess specific changes in the patient’s condition as a result of new or on-going treatments and take immediate appropriate action. •       Implement all TCP Homecare operating procedures and protocols relating to the care and safety of a patient and yourself. •       Ensure a safe clinical environment for patients and yourself by following all procedures and guidelines. •       Attend relevant meetings with company and/or clients and facilitate meticulous feedback. •       Participate in approved data collection, clinical studies and trials as requested by the client. •       Provide support and advice to patient and family member’s clients and multi-disciplinary team. •       To attend all company required meetings, training and other business in a prepared and punctual manner •       Responsible for the administration of daily/weekly/monthly documentation for departmental reviews •       Maintain customer confidentiality at all times •       Represent self in a professional and ethical manner at all times, complying with all applicable company policy and procedures. •       Report all adverse events and product complaints promptly in line with Pharmacovigilance procedures and current legislation. •      Able to educate and communicate clearly while building a good rapport with patients •      Work collaboratively as part of a small, dedicated team •      Maintain records accurately with attention to detail and the return of all patient records and files to head office. •      Liaise closely with all other external/internal personnel associated with the patient programme. •      Attend relevant meetings with Customer/Client to facilitate structured communication flow. •      Keep in touch with current trends and developments in best practice standards and clinical care in this specific area. •      Applicant must be flexible and available to travel Nationwide with some overnight stays   required  Key Competencies:  •      Cannulation & Phlebotomy Skills Essential •      Demonstrable IV therapy experience including care of central venous access devices. •      Effective administrator, with a high level of attention to detail. •      Good time management skills. •      Excellent communication skills, ability to present to a multidisciplinary team. •      Ability to quickly build good rapport with new contacts. •      Likes to work alone yet values their position within a team. •      Demonstrates good sound judgement.•      Demonstrates flexibility with tasks both administrative and clinical to support the service What We Offer in Return:• Competitive annual salary• Company car/phone/tablet• 24 days of annual leave, rising with service• Pension Scheme• Health Insurance• Life Assurance• A supportive and friendly working environment• Regular training courses and opportunities to up skill• Promotional opportunities• Refer a Friend scheme• Social events
