Care Coordinator

Job Title: Care CoordinatorLocation: Dorridge and surrounding areas.Salary: £13.50 per hourWorking Hours: 30 - 40 hours per week, office based and out in the field. We're seeking an exceptional Care Coordinator to join our dynamic team, dedicated to delivering high-quality care and support to our clients in the comfort of their own homes. As a key member of our team, you'll have the opportunity to make a real difference in the lives of our clients, their families, and caregivers. Key Responsibilities:On-Call Duties:Be 'On Call' on a rotational basis to provide emergency relief to cover calls outside of office hoursProvide high-quality care and support to clients in their own homes during these periodsClient Care:Assist with personal care, medication administration, and daily living activities for clientsConduct spot checks, perform risk assessments, and complete client forms as requiredMonitor and report any changes in clients' health or well-being to the Deputy Care ManagerTeam Supervision:Supervise and support a team of care assistants to ensure high-quality care is deliveredProvide guidance and training to care assistants as neededRecord Keeping:Maintain accurate records and documentation of client care, including care plans and updatesEnsure all care plans are completed on the Bright Dawn Home Care computer system and care assistants are notified of any changesScheduling:Schedule rotas alongside the Deputy Care ManagerNotify clients of unforeseen changes in their set rotasCommunication:Communicate effectively with clients, families, and care assistants to ensure seamless care deliveryAdditional Responsibilities:Assist the Deputy Care Manager to cover any uncovered shifts as requiredEnsure all care assistants are aware of any changes to client care plans or rotasRequirements:Driving licence valid for use in the UK and access to your own vehicleExperience in a similar role (care coordination, team leader, senior)Strong communication and interpersonal skillsStrong organizational skills and attention to detailAbility to work independently with minimal supervision Preferred:Experience in a domiciliary care settingExperience supervising a team of care assistantsCertification in care coordination or a related field Proficiency in electronic record-keeping systems (e.g., Bright Dawn Home Care system)What We Offer:Competitive salary and benefits packageOngoing training and development opportunitiesA dynamic and supportive work environmentThe chance to make a real difference in the lives of our clients If you're passionate about delivering exceptional care and support to your clients, we'd love to hear from you!
